Incident Investigation ServicesUnfortunately accidents and incidents do happen. Sometimes when we least expect them. When these things do happen, it is important that prompt and correct action takes place to control, analyze and improve so that they do not happen again. Our experienced staff can help you cover all the right bases, look in all the right places, and determine the best method of correcting the outage that caused the incident in the first place.
Legal Documentation The correct legal documentation is necessary up front, to determine the WHO, WHAT, WHEN, WHERE and HOW of the incident. Getting this done completely up front, as soon as possible, gets the information when it is fresh. Not two years later when some attorney is now pressing for answers or when the involved employees have all moved on to the next job. Remember the saying, “If it is not in writing, it did not happen.”
Root Cause Analysis Getting the REAL Root cause of an incident is important if you really want to stop that incident from happening again. Many investigations often stop once they have determined whom to blame, whether that is management or one of the employees. But it is important to go to the next step and find that root cause. If not, other times companies continue to do the same training, hire employees and train them in the same manner, and supervisors direct employees in the same way, BUT EXPECT different results. To get different results, you have to change what it was that caused the incident.
Action Plan Development and Implementation Once a root cause or causes have been identified, then corrective action is needed. Our staff can make some recommendations on how to improve your systems so that it does not happen again. We can also help to improve those systems through training, education, and auditing so that the action plan is effective. |
